# Form

{% hint style="info" %}
Forms are available on our **Growth** and **Enterprise** plans.
{% endhint %}

### How do I add a form to my demo?

In <kbd>**Edit**</kbd> mode, click <kbd>Form</kbd> in the toolbar to add a form to your demo, then customize the form fields.

To add a form field, click <kbd>+ Add Field</kbd> in the form settings.

### Form Settings

**Type:** You can set the form fields to collect **emails**, **text**, or **multiple-choice responses**.

**Field Name:** This field is not visible to viewers and is used internally to define the column name in your demo analytics.

**Required:** Whether or not the viewer can continue without completing this field.

**Validation rules:** This lets you filter out generic email addresses (e.g., Gmail) and only accept company emails.

### Where can I view the form submissions?

1. **Forms page:** Navigate to **Forms** in the left sidebar account and select the form from the table.
2. **Analyze tab:** To view a specific demo's form submissions, go to the **Analyze** tab of that demo, and scroll to the Form Submissions section.

### Can I download the form submissions?

Absolutely! To download the form submissions, navigate to the **Forms** page, select the desired form from the table, and click on <kbd>Download CSV</kbd>.
